Use this form to send your condolences to a family.  Include as much information as you would like.  The information will be printed onto a condolence card and given to the family.  Click here for an example of the card.  Whether it be a current death or one for some time back, we will see that it gets to the family.  Or if you prefer to, here are some guidelines to help create and send your own condolence.

If it is meant for a specific family member be sure to include who.  If you are unsure of the deceased's name enter "unsure" in the For the family of field and include the full name of the relative you know and we will get it to the right family.
